The Annual Wellness Visit (AEV) is a once-a year, prevention-focused visit between a patient and a primary care provider. It is a valuable opportunity to build strong relationships with patients, identify and document health risks, and complete a number of important preventative care activities. These visits allow providers and patients to update information about a patient’s health status, set goals and a preventative care plan for the year ahead, and close any gaps in care.

Primary care providers are the cornerstones of patient care, particularly in adults age 65 and older. Research shows that people who have an ongoing relationship with a primary care provider and get their recommended screenings and immunizations have better overall health and lower mortality rates.
